ADMISSION
REQUIREMENTS
Enumerated below are the admission requirements for each school in the academy:
PRE-SCHOOL
At least five (5) years old for Kinder on or before October 31, 2023
Photocopy of PSA (NSO) birth certificate;
2pcs 2x2 id pic (white background)
ELEMENTARY SCHOOL
Photocopy of PSA Birth Certificate
At least six (6) years old for Grade 1
Original copy of Report Card/Form 138 (for those entering in Grade 1 to 6)
Form 137/ School Form 10 (Permanent Record) from previous school
Certificate of good moral character (Grades 4-6) and has not been expelled from other school or dismissed for disorderly conduct
An applicant must be a graduate of the previous grade level or its equivalent. Proofs of equivalency should be submitted for assessment
JUNIOR HIGH SCHOOL
Photocopy of PSA Birth Certificate
Original copy of Report Card/Form 138 (for those entering in Grade 7 to 10)
Form 137/ School Form 10 (Permanent Record) from previous school
Certificate of good moral character (Grades 7-10) and has not been expelled from other school or dismissed for disorderly conduct
